Is a gear-less cruiser bike challenging enough for exercise?
My physical therapist told me I should start riding a bike to help my knees (seriously flexible joints). Should I just stop being cheap and opt for more gears or is a simple bike good enough?.

A geared bike is the only way to go. When you hit something strenuous, you can down shift to a lower gear. With a single gear... you walk or cry.
I am bone -on- bone in my left knee, and have been for a decade. I can still ride great distance daily on my road bike. The only time I feel pain is when I hit a steep hill. Then the pedal pressure gets to me.
In the case of flexibility issues, you want to make sure that the bike is set up properly... especially for seat height. You want to be able to turn over the pedals in 'circles' without having to rotate your hips. This may take some time, so go back in and have it adjusted as the knee gets more flexible until it is at the correct height.
